Home Care, Aged & Disability Support Worker

The Home Care, Aged & Disability Support Worker provides essential one-on-one assistance to older Australians and people living with disabilities. The primary focus of this role is to deliver person-centred care that promotes independence, dignity, and community inclusion, enabling clients to live safely and fulfilling lives in their own homes. KEY RESPONSIBILITIES • Personal Care & Hygiene: Assist with daily living activities including showering, bathing, grooming, dressing, and oral hygiene while maintaining the client’s privacy and dignity. • Domestic Assistance: Perform general household tasks such as light cleaning, laundry, bedmaking, and sanitising living areas. • Meal Support: Assist with menu planning, grocery shopping, and the preparation of nutritious meals according to dietary requirements. • Community & Social Participation: Provide transport and accompaniment to medical appointments, social outings, shopping trips, and recreational activities to prevent social isolation. • Health & Medication Monitoring: Assist with medication administration (within scope of training) and monitor for any changes in physical or mental health, reporting concerns promptly to supervisors. • Documentation & Reporting: Maintain accurate and confidential progress notes, shift handovers, and incident reports in accordance with organisational policies and NDIS/Aged Care standards. • Mobility Support: Assist with safe transfers and the use of mobility aids (e.g., hoists or wheelchairs) following proper manual handling procedures REQUIREMENTS OF THE JOB • To be available and attend all allocated shifts to you through the Organisation rostering system. You must notify the office immediately if you identify you are unavailable for any reason to undertake shifts on your rostered days. • To frequently review and monitor your roster to ensure you are aware of all changes applicable. • To ensure all client case notes and client information is updated during each interaction or shift with clients. • To report any incidents that occur on shift immediately to the office by calling 02 6424 7108 and to complete an online incident report form via the rostering application (app) prior to ending that shift. • Maintain a clean and safe workspace, including your own vehicle. • To actively adopt the Vision, Mission and Values in everyday work. • Abide by all the Organisation policies and procedures. • Stay up to date with all relevant policy and procedures relating to your role. • To monitor your emails and texts on any day you work to ensure you receive important updates from the company. • Attend all individual, supervision and team meetings, development and training days as requested by the employer. • Commit to ongoing development and training and to meet your ongoing training, quality, safety and development needs. • Have a clean, smoke free clean and tidy vehicle that is road worthy, comprehensively insured and is able to meet the needs of the clients. To ensure your vehicle has fuel for your day prior to commencing work, it is not appropriate that a worker uses a client shift time to refuel unless on a trip out of area (some large or hard to access vehicles due to height and other reasons may exclude some clients from accessing your vehicle, you agree to continue to have full use of a suitable vehicle for client transport). • Other tasks as directed by your immediate supervisor, the administration and rostering team or any other authorised personnel of the employer

ESSENTIAL SKILLS & QUALITIES • Empathy & Compassion: A genuine desire to support others and the ability to build trusting, respectful relationships. • Communication: Strong verbal and active listening skills, including the ability to adapt communication for clients with sensory or cognitive impairments. • Patience & Resilience: The ability to remain calm and supportive in unpredictable or challenging situations. • Reliability: Commitment to showing up on time and following care plans consistently. • Cultural Sensitivity: Respect for diverse backgrounds, beliefs, and "people-first" language. • Experience working with vulnerable people in a role which would allow transferable skills or appropriate lived experience. • A passion for supporting people with a disability/health condition that impacts their capacity to have the best life they possibly can, by considering clients full human rights and making them feel valued as part of their local community. • Demonstrated capacity to work with honesty and integrity, • Good literacy and numeracy skills. • Practical written and verbal communication skills, including writing comprehensive daily case notes and the capacity to apply policy and procedures in your work. • Organised and able to meet deadlines. • Solid computer and technical skills with the ability to use online platforms QUALIFICATIONS & REQUIREMENTS Mandatory: • Possession of a NDIS Worker Screening Check linked to the Organisation (renewable every 5 years at workers cost) • Possession of a National Criminal Record Check (renewable every 3 years at workers cost) • Possession of a cleared Working With Children Check (Paid worker) every 5 years renewal period • Possession of an existing First Aid (every 3 years) and CPR (every 1 year) • Current and valid NSW Driver’s Licence • Possession of Comprehensive Vehicle Insurance for a reliable • Completion of all the NDIS Worker Modules • Certificate III in Individual Support (Ageing or Disability) or actively studying to complete • Possession of all health-related vaccination requirements as per the Contract requirements for workers undertaking client care with NSW Health and to keep those requirements up to date. • Experience in manual handling and using assistive technology
